Established in 2020, Elevate CCBHC started as recipients of a two-year federal Substance Abuse and Mental Health Services Administration (SAMHSA) grant to expand quality health care to individuals without insurance and those underinsured. We use a “no walls” approach to provide behavioral health responses to anyone anywhere in Black Hawk County and surrounding areas. We are committed to providing our community with resources and services to identify your strengths and help you build a more healthy and happy life. Elevate offers behavioral health services and crisis services.
